SPECIALIST OCCUPATIONAL THERAPIST OR PHYSIOTHERAPIST 27.5 HOURS - OTHER PART TIME HOURS CONSIDERED
The Discharge to Assess team is an innovative and exciting service, aiming to facilitate timely discharges to the residents of Stockport and surrounding areas. Working as part of a multi-disciplinary team, they will facilitate timely discharges from a non-acute community bed and provide assessment and support planning to meet a person’s needs in the community.The post holder will provide and/or arrange appropriate support interventions in the community via Discharge to Assess collaboratively working closely with colleagues across all sectors.
Working as part of a lively and dynamic multi-professional team, you will deliver holistic support and interventions to improve the health and well-being of individuals in a bed based facility. Main duties of the job
We are looking for highly motivated, forward thinking and experienced clinicians with a specialist interest in frailty who will work alongside an integrated Multidisciplinary Team to improve flow and quality of the patient's journey. You must have excellent communication, organisational and leadership skills, with the ability to work autonomously, whilst being flexible and adaptable to supporting the service leads in developing and changing the service as it evolves.
You will be part of an integrated therapy team and we have a strong commitment to support all staff members with CPD opportunities. We offer regular supervision and annual appraisal with a regular in-service training programme.
The post requires a commitment to weekend working which is underpinned by teaching and a competency programme.
The post requires working hours over a 7 day service.

Detailed job description and main responsibilities
Qualified Occupational Therapist or Physiotherapist.
The Discharge to Assess team continually develop and enhance the seamless transfer of care from a D2A community bed to a patient's usual residence. The successful candidate will therefore embody trust values, in supporting and facilitating safe discharge to individuals who are deemed medically optimised.The successful candidate will be passionate in providing community based interventions, demonstrating a high level of holistic care independently and as part of a team.
The Multi professional team consists of highly skilled, registered Nursing staff, Occupational Therapists and Physiotherapists, Pharmacy and Discharge Coordinators. Complementing our registered staff, we excel in providing a large team of Health Care Assistants, Admin, Discharge Trackers and Therapy Assistants.As a valued member of the team, you will be expected to liaise with colleagues across all sectors to facilitate discharge but also work independently to plan care and implement from a patient centred approach.
The team provides a seven day service, therefore this entails working some weekend and bank holidays. The post is community bed based delivering assessments and interventions where required; therefore independent travel with use of your own vehicle, business insurance and a full driving licence to deliver the role is essential.
We promote a positive ethos on personal and professional development, providing access to training and supervisions. You will be offered induction, supernumerary working and full access to mandatory training, furthermore CDP training is full supported and very much encouraged.Regular PDRs and reflective supervisions are provided, alongside peer support sessions in the form of multidisciplinary and profession specific team meetings.
If successful this vacancy will require an enhanced DBS check at a cost of £54.50. This cost will be deducted from your first Trust salary
In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.

Education & Qualifications
Essential criteria- • Diploma/Degree in Occupational Therapy.
- • State Registration with the Health & Care Professions Council.
- • Commitment to lifelong learning – evidence of CPD maintained in a portfolio.
- • Experience as clinical educator.
Knowledge
Essential criteria- • Specialist knowledge and application of occupational therapy assessment and interventions.
- • Ability to use and interpret a range of standardised assessment.
- • Use of OT outcome measures
- • Basic I.T. skills.
- • Appraisal Skills training.
- • Clinical Supervisor training.
- • Introducing management training.
- • Clear vision of role/commitment to specialty.
- • Evidence of some area of special interest.
Experience
Essential criteria- • Post-registration experience of working with a variety of conditions.
- • Supervision of junior staff, assistants and students.
- • Involvement in relevant audit.
- • Leadership and first hand management experience.
Skills & Abilities
Essential criteria- • Extensive clinical experience in OT.
- • High level of rehabilitation skills.
- • Ability to work autonomously and set own priorities.
- • Ability to reflect and critically appraise own performance.
- • Ability to provide spontaneous and planned solutions to complex problems.
- • Ability to pass on skills/knowledge to others within both formal and informal environments
- • Ability to communicate with patients where there may be significant barriers to understanding.
