Managerial responsibilities: to include level of responsibility for: Management and further development of the Health Inequalities, Population Health Management and Prevention work programme, ensuring a system approach. Drive the delivery of associated projects at pace.
Develop appropriate work packages and lead service improvement and transformation projects and/or design innovative services, in conjunction with project management resource, that look to address health inequalities, tackling the wider determinants of health, taking a population health management approach.
Responsible for increasing the knowledge and understanding of the health inequalities, Core20PLUS5 and population health management agendas with internal and external partners, including staff within Primary Care Networks. Develop and nurture strong, collaborative and positive relationships with external partners outside of health, to support the reducing health inequalities agenda and embed a population health management approach to all our work.
This will include, but not be limited to Councils, Public Health, Local Authority and Voluntary Care Sector organisations. Lead relevant working groups and provide programme and project advice, subject matter expertise and support, including the coordination of these groups, as requested.
Responsible for ensuring the projects within the programme have involved co-production with a wide range of stakeholders. Lead successful implementation of the overall programme objectives and report on progress to the DGS HCP and other committees and groups as appropriate, including the Inequalities, Prevention and Population Health Steering Group, Trust Leadership Team Meeting and the Clinical and Professional Senate.
Lead members of project teams in investigating the causes of any variance from plan and/or areas of health inequality and the implementation of effective remedial solutions. Lead a dynamic and appropriate approach to risk management through the use of Risk & Issues Logs, including mechanisms and resolution, feeding into Directorate, PMO and Corporate processes, as required.
