Dartford, Gravesham and Swanley (DGS) Health and Care Partnership (HCP) is a partnership of NHS organisations, councils, voluntary care sector representatives, public health and local authority organisations. The HCP representatives work collaboratively to design and deliver better integrated services to meet the needs of the population they serve, with a particular focus on areas of greatest need.
The purpose of this role is to support the embedding of a Population Health Management (PHM) approach across PCNs and wider stakeholders across the DGS footprint with the aim of reducing Health Inequalities (HI). The post holder will also be required to work closely with Public Health, local Council colleagues and the voluntary sector, to implement community engagement activities, developing local initiatives that help support our most vulnerable population.
This role is one of four PHM Programme Manager posts, each based within one of Kent and Medway’s Health and Care Partnership areas: Dartford, Gravesham and Swanley (DGS); East Kent; Medway & Swale and West Kent. There will be an expectation to work collaboratively with those personnel as well as link with the relevant Kent & Medway (K&M) ICB leads.Addressing health inequalities through PHM will be a key focus of the work.
Main duties of the job- Using a data driven approach, identify and tackle key areas of health inequalities within DGS, applying population health management techniques to priority areas.
- Operationally develop, lead and facilitate the Health Inequalities & Population Health Management programme and associated projects, including all relevant programme management and delivery plan documentation and reporting.
- Ensure appropriate management of programme objectives and outcomes, identifying and managing risks, issues and dependencies, considering best practice and making decisions in the best interest of the programme.
- Responsible for implementing quality improvements and changes locally at a DGS HCP level through collaboration with all relevant system partners.
- Operate effectively in a flexible and demanding environment and proactively engage with staff across all key stakeholders within the HCP within DGS.
- Present highly complex information about projects, initiatives and services to a wide range of stakeholders in a variety of formal and informal settings through written and verbal reporting.

Detailed job description and main responsibilities
Managerial responsibilities: to include level of responsibility for:
Management and further development of the Health Inequalities, Population Health Management and Prevention work programme, ensuring a system approach.
Drive the delivery of associated projects at pace.
Develop appropriate work packages and lead service improvement and transformation projects and/or design innovative services, in conjunction with project management resource, that look to address health inequalities, tackling the wider determinants of health, taking a population health management approach.
Responsible for increasing the knowledge and understanding of the health inequalities, Core20PLUS5 and population health management agendas with internal and external partners, including staff within Primary Care Networks.
Develop and nurture strong, collaborative and positive relationships with external partners outside of health, to support the reducing health inequalities agenda and embed a population health management approach to all our work. This will include, but not be limited to Councils, Public Health, Local Authority and Voluntary Care Sector organisations.
Lead relevant working groups and provide programme and project advice, subject matter expertise and support, including the coordination of these groups, as requested.
Responsible for ensuring the projects within the programme have involved co-production with a wide range of stakeholders.
Lead successful implementation of the overall programme objectives and report on progress to the DGS HCP and other committees and groups as appropriate, including the Inequalities, Prevention and Population Health Steering Group, Trust Leadership Team Meeting and the Clinical and Professional Senate.
Lead members of project teams in investigating the causes of any variance from plan and/or areas of health inequality and the implementation of effective remedial solutions.
Lead a dynamic and appropriate approach to risk management through the use of Risk & Issues Logs, including mechanisms and resolution, feeding into Directorate, PMO and Corporate processes, as required.

Education and Training
Essential criteria- Educated to Masters Degree level, eg: in Business Management OR equivalent experience
- Evidence of management and leadership development
- Project Management qualification or evidence of formal training
- Evidence of continuous professional development
- Educated in Population Health Management and/or Public Health
