The Post Holder must have the individual service users needs at the fore at all times and use the agreed set of values and skills to underpin their day to day work. To work without direct supervision, but with someone available to refer to, as and when required, either on or off site on a regular basis, Ensure that service users engage effectively with the agreed care plan and participate in community support on a regular and consistent basis.
Provide regular and practical support to service users and their carers/significant other in developing and managing dignity and independence. Provide support and assistance, as necessary, with daily living activities. Positively promote independent living skills of service users.
Work flexibly with Care Co-ordinators, other relevant community support, service user and significant other to implement plan of care. Help service users gain access to resources, including benefits and welfare rights. Will be handling patients cash and valuables on a regular basis.
Help identify early signs of relapse by monitoring the services user progress, level of functioning and mental state and alert the appropriate staff involved in the persons care. Assist the Care Co-ordinator and service user in developing crisis/contingency plans and advanced statements, as per local policy guidelines.
To maintain adequate records, as required, by existing procedures, entering appropriate details on the service users electronic record and any paper held files. To be able to communicate routine information in an arena where there will be barriers to understanding, and to be able to use tact and persuasion in a non-aggressive manner.

Attend and participate in agreed training, development and supervision sessions as an individual and as part of the team. To undertake such other duties as may be determined from time to time within the general scope of the post. Adhere to health and safety legislation
