Main duties of the job
Oxleas provides a range of mental health and psychical health services across Southeast London and forensic services within the Southwest of England. This post will support community services for the Royal Borough of Greenwich.
Our UCR service known as the Joint Emergency Team (JET) and it is a specialist urgent service for people who have suffered sudden acute deterioration in their health or functional needs. This multidisciplinary team receives acute referrals from LAS, 111, ED, and GPs.The team will visit patients at home to carry out a comprehensive geriatric assessment, discussing the diagnosis and management plan with the Consultant/GP and treating the patient at home to prevent unnecessary admissions to hospital.
JET is also responsible for the Greenwich (virtual wards) VWs which allow people to receive hospital care at home. The VWs prevent admissions by treating appropriate non-emergency medical problems at home and providing physiotherapy, occupational therapy and social services input where necessary.The VW team also supports ED and inpatient early supportive discharges. The use of technology-enabled care is well embedded in the team. In conclusion, the work of this team is in line with the government’s 3 shifts: from hospital-to-community, from treatment-to-prevention, and from analogue-to-digital.

Detailed job description and main responsibilities
This job description is for a full-time 10 PA postholder. Should any applicant wish to undertake a part-time position, the clinical duties and job plan will be adjusted accordingly. There is no on-call responsibility attached to this post.
The duties of the postholder will include:
- Consultant/GP leadership for JET and the Greenwich VW Team
- Participation in quality assurance and audit projects
- Clinical review of all JET and VW patients though direct assessment or via team board rounds and multidisciplinary meetings
- Clinical supervision of the ACPs in their teams
- 1.5 PA of supporting professional activities for audit work, service development, structured teaching and CPD
- No on-call commitments
- The descriptions of the post cannot be exhaustive, and the post-holder may be required to provide Consultant/GP support across the wider community service provision following discussions with the Clinical Director or Service Director, normally these will be broadly in line with the listed key responsibilities.
Interested applicants will be able to visit the site and speak to the Deputy Chief Medical Officer (the Clinical Director post is currently vacant) and operational colleagues.
The postholder will be working closely with the service manager, the Clinical Director, and other key nursing staff to ensure high quality clinical care for the patients and ensure that the service model undergoes continuous improvement utilising the available resources.
