Social Worker

You will be:

Joining the Adult Integrated Hospital Discharge Team; your role will be to:

• Cover intermediate care bed-based discharge work.
• Work within a multidisciplinary team to ensure residents receive a safe and timely discharge.
• Completing assessments and ensuring a care plan is in place.

Essentials:

• A Social Worker Bachelor’s Degree and an ASYE completed.
• The ideal Candidate will have direct experience of working within Adult Social Work, it would be beneficial if you have previous experience of working within an Adult Hospital Discharge Team.
• Registered to Social Work England.
• Right to work in the UK.
