A patient’s recovery can be impacted by staying in hospital longer than is necessary so having the right care plan in place to support the person to leave hospital at the earliest and safest time for them is crucial.

The Acute Trust Case Managers support the effective management of patient discharge from hospital and play a pivotal role in supporting ward-based colleagues with patient discharge plans.

The Acute Trust Case Managers work with all members of the newly formed Transfer of Care Hub (ToCH) based at the Weston General Hospital , including social workers, Sirona CToCH (Community Transfer of Care Hub) team and community therapists and will be encouraged to participate and share learning with colleagues across the team.

We are looking for Registered Nurses or AHPs (Allied Health Professionals) to join our team.
