We are looking to recruit a Registered Nurse with both Clinical and Management experience, who will have responsibility for the day to day management and clinical leadership of the Urgent Community Response & Virtual Ward team across East Berkshire

The role involves working closely with service users, community-based community services, Primary care, other community health services and social services and the voluntary sector to ensure that the services provided is of a high quality, responsive to local need and demonstrates value for money

You will be a team player and work collaboratively with staff from all disciplines whilst maintaining high professional standards. You will ensure good communication, and effective processes and procedures are maintained across the service.

The post holder will work 37.5hrs per week, 7.5 hours a day, Monday to Friday between the hours of 8am – 8pm.

The role is based at St Marks Hospital and free staff parking is available

Main duties of the job
  • To practice at an advanced level of nursing, working as an autonomous practitioner within the multidisciplinary team including admitting to and discharging patients from the Urgent Community Response Service Virtual Frailty Ward in collaboration with the UCR VFW Geriatrician and Specialty Doctor
  • As an experienced nurse practitioner, you will take responsibility and be accountable for the clinical assessment, diagnosis, treatment and management of complex patients considering local and national guidelines.
  • The postholder is expected to liaise effectively and professionally with all levels of staff, external organisations and the public, dealing with confidential information in an appropriate manner
  • To provide leadership and line management to the Band 8/7/6 nurses and therapists and the Band 4 healthcare/multi-therapy assistants.
  • Ensure accurate, contemporaneous documentation and care plans are maintained accordance with the NMC, information governance and local / national guidance.
  • Work collaboratively with colleagues and develop nursing and care delivery policies, identifying how policy will impact on practice and service delivery.
  • Work flexibly in response to service need across our multiple sites

Education/Qualifications/Training

Essential criteria
  • Educated to a postgraduate or masters level
  • NMC registration
  • Advanced History taking and assessment
  • Additional qualification in management related field or equivalent experience
  • Evidence of continual professional development
Desirable criteria
  • Non-medical Prescriber.
  • Diagnostics reasoning module

Previous Experience/Continuing Professional Development

Essential criteria
  • Substantive experience of leading and managing a team, including performance management, sickness monitoring, recruitment, induction, appraisal, skill mix and staffing rotas
  • Ability to work under pressure, time managements skills to prioritise work.
  • Previous experience of effective stakeholder engagement
  • Comprehensive range of supervisory /coaching experience both clinically and managerially
  • Previous experience of multi-disciplinary team and partnership working
  • Identification of local health needs and translating this into the people and finances needed to meet the need
Desirable criteria
  • Previous experience of change management and experience of leading service change
