We are looking to recruitaRegisteredNursewith bothClinical and Managementexperience,whowill have responsibility for theday to daymanagement and clinical leadershipofthe Urgent Community Response & Virtual WardteamacrossEast Berkshire
The role involves working closely with service users, community-based community services, Primary care, other community health services and social services and the voluntary sector to ensure that the services provided is of a high quality, responsive to local need anddemonstratesvalue for money
You will be a team player and work collaboratively with staff from all disciplines whilstmaintaininghigh professional standards. You will ensuregood communication, and effective processes and procedures aremaintainedacross the service.
The post holder will work37.5hrs per week, 7.5 hours a day, Monday to Friday between the hours of 8am – 8pm.
The role isbased atSt MarksHospital andfreestaff parking is available- To practiceat an advanced level of nursing, working as an autonomous practitioner within the multidisciplinary team including admitting to and discharging patients from the Urgent Community Response ServiceVirtual Frailty Wardin collaboration with theUCR VFWGeriatricianandSpecialty Doctor
- As an experienced nurse practitioner, you will take responsibility and be accountable for the clinical assessment, diagnosis,treatmentand management of complex patients considering local and national guidelines.
- The postholder is expected to liaise effectively and professionally with all levels of staff, externalorganisationsand the public, dealing with confidential information inan appropriate manner
- To provide leadership and line managementto the Band8/7/6nurses andtherapists andtheBand4healthcare/multi-therapy assistants.
- Ensureaccurate, contemporaneous documentation and care plans aremaintainedaccordance with the NMC, information governance and local / national guidance.
- Work collaboratively with colleagues and develop nursing and care delivery policies,identifyinghow policy willimpacton practice and service delivery.
- Work flexibly in response to service need across our multiple sites

The “must haves” for this role:
- Advanced Practitioner registration or working towards obtaining registration
- Professional health registration (RN)
- Mastersdegree or equivalent experience
- Clinical andLeadership experience
- Able to work autonomously, with the ability to work under pressure, with excellent organisational, communication and time management skills to prioritise work
