The Physiotherapy department are looking to recruit a driven and ambitious experienced band 5 or existing band 6 therapist to work within our Hospital Outreach Support Team (HOST).

HOST is a unique and highly regarded team of physiotherapists and occupational therapists, established to facilitate acute discharge from hospital and help reduce risk of readmission for complex patients. HOST accept referrals from all specialities and demographics, ensuring the caseload remains varied.

This is a permanent, full-time position and the successful candidate will demonstrate excellent multidisciplinary team working, adaptable communication for complex patients, be able to problem solve autonomously and be innovative in their approach to complex patients.

You must have experience within the NHS, preferably having completed rotations of the acute setting and also be a car driver/owner. There is a requirement for flexible working as the service currently covers a 7 day working week, including bank holidays, using a rostered system.

Main duties of the job

By reviewing patients in their own homes immediately following discharge, HOST provide short term patient centred rehabilitation, adaptive aids and advice to maximise opportunity for patients to reach their functional potential and remain safely at home.

In addition, HOST also support patients on discharge with management of a variety of orthopaedic devices, including cervical collars and upper and lower limb braces.

Detailed job description and main responsibilities

To provide physiotherapy to relevant patients within HOST, in a range of settings including inpatients, outpatients and patient homes.

To assess and treat patient’s in HOST. To support the team in the organisation and provision of an efficient, high quality and co-ordinated physiotherapy service for patients in each area. To undertake a defined clinical caseload. To ensure that patient and service standards and priorities are met.

To provide clinical supervision to assigned junior physiotherapists, technical instructors, physiotherapy assistants and students. A rotation with inpatient services may be considered.

Qualifications

Essential criteria
  • HCPC registration
  • BSc Physiotherapy
Desirable criteria
  • Member of CSP
  • Member of Specific interest group
  • Post graduate qualifications in related area i.e. frailty

Knowledge

Essential criteria
  • Previous Acute NHS experience at band 5 level
  • Achieved core skills in all core Junior rotations, including elderly care and orthopaedics
  • Evidence to support knowledge base in specialist area
  • Previous acute NHS experience
Desirable criteria
  • Previous Senior Experience in Specialty Area
