Care Coordinator - Supported Living Bedford Safehands are looking for an experienced Care Coordinator to join their team in the Bedford area. The service is based in a beautiful, rural location and has 4 bedded residential service. The service has been designed for 4 users with complex needs.

Assessing clients' care needs and developing personalised care plansCoordinating and scheduling care services for clientsMonitoring and evaluating the effectiveness of care plansManaging and Coordinating the weekly rotasCompleting weekly payroll data inputEnsuring compliance with regulatory standards and guidelinesMaintaining accurate and up-to-date client recordsRequired:
Previous experience in a care coordination or similar role is desirableKnowledge of healthcare systems and proceduresExcellent communication and interpersonal skillsStrong organisational abilities to manage multiple tasks efficiently
