Duties & Responsibilities: To care for the person and their family holistically i.e. recognising the importance of physical, emotional, social and spiritual aspects of care. To work unsupervised in the patients home. To undertake simple dressings independently and assist the District Nurse/ Registered Nurse with more complex dressings.
To report back to SinglePoint any symptoms, concerns, changes or deterioration of the patients condition. To take advice / instructions from St Helena Medical and Senior Staff on-call when necessary. To communicate effectively with members of the interdisciplinary team, District Nurse, GP, Social Services etc.
To maintain accurate records of patient care in accordance with St Helena policy. To ensure confidentiality of information at all times, in both written and verbal form. To have awareness of the function and use of equipment used in the care of the palliative/end of life patient at home.
To adhere to St Helena uniform policy To attend regular staff meetings and professional palliative care updates as well as other statutory updates. To attend clinical supervision sessions. To participate in educational programmes carried out within St Helena, as both provider and recipient.
To work in other areas if required (IPU / Day Services) as instructed to by the Service Manager or member of senior staff. To visit a post-bereavement family at home if required (as part of the post-bereavement care). To provide emotional support and closure if required.
