CYP Continuing Care Team Manager

To work in partnership with CYP and their families the post holder leads the implementation of the Childrens Continuing Care Framework to include personal health budgets and fast track in line with national policy, guidance and frameworks to ensure the ICB is fully compliant with legal and statutory requirements.

To ensure that care packages and placement proposals are clinically appropriate, person centred, have all the agreed documentation in place, and are making best use of agreed resources to meet the agreed outcomes To work closely with partner agencies and the local authorities to continue to support the implementation and development of jointly resourced packages of care, ensuring that the child's needs are met safely when these have been agreed and the responsibility of health.

To takea lead role in coordinating the management of complex cases across the health community,ensuring at all times that patient choice, safety and quality remain uppermost in informing decisions. Ensure appropriate engagement with patient, family members and key stakeholders.
To advise and support, children, young people, their families and carers throughout the process of assessment and case management of support packages. To complete personalised health assessments for children and young people and chair multi-agency meetings to inform the decision making process within the Continuing Care Framework.
To attend and contribute to the Childrens Continuing Care eligibility panel. To participate in Care and Treatment Reviews (CTRs) for CYP in tier 4 provision. To monitor contracts with continuing care and complex health care providers ensuring monthly reports are received and reviewed.
To undertake an annual quality visit of all children and young people who have a continuing care package as a minimum. To ensure the delivery of a high quality, responsive service to meet the health needs of the identified population. To prepare for the transition of young people with care packages moving from children to adults services.
Ensure accurate, open communication and co-ordination with a range of organisations and individuals. Research and draft correspondence and papers and ensure the management of specific tasks, lead reporting and analysis across a range of specialties, functions and projects.
Be a key member of the team as well as supporting effective communication and stakeholder management, both internally and externally. To provide contemporaneous records and reports adhering to NMC standards for record keeping. To develop commissioning knowledge with the support of the children and young peoples commissioning team.
Act in an educational and advisory role to primary care staff and other partner organisations to promote and share knowledge concerning complex health care. To attend national events in relation to the role and represent the ICB in the evolving agenda in relation to all elements of the role.
