Job overview

The post holder will provide leadership and project management support in a locality as well as working with partners across the health and care system in BSW to ensure patients receive their care and treatment wherever possible in the community.

The role will also play a key role in the development and interpretation of local, regional and national datasets and dashboards to assist in the understanding of local performance.

Main duties of the job

This role will support services commissioned through the Better Care Fund (BCF) and other funding streams such as winter pressures, but this portfolio will flex according to the priorities of the locality in pursuit of developing an integrated care alliance in each locality and the development of the BSW Integrated Care System (ICS).

Detailed job description and main responsibilities

Key Working Relationships

The post holder will be required to work in a network way with other members of the community health and care, complex and specialist commissioning teams.

The post holder will have regular contact at a senior level with internal and external partners across BSW and will often need to engage with them over sensitive, complex and confidential issues.

The post holder will participate in relevant internal and external working groups which support the delivery of integrated pathways of care.

The post holder will need to be able to present and communicate complex information to a range of internal and external stakeholders.

The post holder will work in the matrix way connecting with a variety of services across BSW.

Operational Responsibilities

The post holder will work in a network-style with other members of the team to support the delivery of integrated health and care services in their locality, as well as supporting the Head of System Flow and the other localities within the BSW ICB.

The post holder will have oversight of performance against flow and discharge to include long length of stay and work with system partners to ensure that people do not remain in hospital when clinically fit to be discharged.

The post holder will support demand and capacity modelling work work that will enable us to maintain flow through seasonal pressures and any other increase demand.

The post holder will be the lead for winter planning (including other bank holiday periods) for the locality working with other equivalent roles and leads in BSW.

The post holder will take the lead on specified flow priorities that may vary from time to time, chairing or facilitating any task and finish groups required to support the delivery of the work programme.

The post holder will support and take forward specific work on behalf of the BSW Urgent Care & Flow Board working alongside the BSW Head of System Flow as required.

The post holder will need to develop project plans, identifying risks, issues and dependencies, considering best practice and current options and ultimately making decisions in the best interest of the project.

The post holder will pro-actively work with partners and stakeholders and be able to respond to and resolve any conflict when this arises through facilitation or other appropriate mechanisms.

The post holder may be required to support flow outside of normal working hours in times of escalation or a surge in demand.

Financial and Physical Resources

The post holder will need to co-ordinate and compile business cases working with partners to secure funding to support the improvement or changes to the way services are delivered. This will include evaluating value for money, their impact and delivery of outcomes reporting to various forum.

The post holder will act in a way that is compliant with standing orders and standing financial instructions in the discharge of budget management responsibilities.

The post holder will need to validate and authorise invoices for applicable services/contracts for payment.

Staff Management

The post holder may have management responsibility and will be expected to provide clear leadership and direction, maintaining excellent communication channels, encouraging collaborative and flexible approaches with other colleagues and teams in the locality and across BSW.

The post holder will oversee work of any project streams, reviewing resource allocation and ensuring any projects are successfully delivered within prescribed timescales that relate to flow.

The post holder will promote all health, safety and welfare issues, and fulfil management responsibilities designated in the health and safety policy.

The post holder will support, motivate and develop other colleagues in the team.

Information Management

The post holder will be responsible for drafting reports summarising status on issues, appraising outcomes, and providing progress reports as required.

The post holder will collate as required, qualitative and quantitative information and lead appropriate analysis to develop robust business cases.

The post holder will analyse, interpret and present data to highlight issues, risks and support decision making.

Policy and Service Development

The post holder will be responsible for proposing and drafting changes, implementation and interpretation to policies, guidelines and contracts which may impact service delivery.

The post holder will need to maintain a good knowledge of emerging policies, strategies and guidelines from NHS England/Improvement, the Department of Health & Social Care, Local Government Association, NICE, clinical networks and senates.

Planning and Organisation

The post holder will contribute to the development and implementation of BSW ICS’s operational and strategic plans, providing the required content and input relating to urgent care from across the locality.

The post holder will utilise expertise and ‘commissioner intelligence’ to interpret requirements and design meaningful outcome measures and ensure their inclusion in service agreements and contract terms, liaising with the Head of Contracting & Performance and the SCWCSU.

The post holder will drive and lead development and improvement of processes.

The post holder will ensure service user, carers and parents’ voice and community engagement is an integral part of the ways of working. Directly address larger forums and handle matters involving controversy over service plans or provisions.

The post holder will develop local partnerships bringing people together to work collaboratively and provide the context for working together.

Knowledge, Training and Experience

Essential criteria
  • Educated to degree level or equivalent level of experience of working at a senior level.
  • Comprehensive commissioning knowledge and experience.
  • Evidence of continuing professional development.
  • Sound knowledge and understanding of the NHS and/or social care context and structures and their implications for commissioning.
