THE OPPORTUNITY This role will be hosted by a GP practice within the Primary Care Network (PCN), and will be working in LS25/26 GP practices. LS25/26 PCN has a patient population of circa 75,000 across 7 practices: Garforth Medical Centre Gibson Lane Practice Kippax Hall Surgery Lofthouse Surgery Moorfield House Surgery Nova Scotia Medical Centre Oulton Medical Centre We are a dynamic, forward thinking PCN team who are passionate about developing and delivering excellent quality local services to meet the needs of our patients.

We have a team of over 40 staff including pharmacists, pharmacy technicians, trainee pharmacist, paramedics, social prescribers (general and working with young people), Health Care Assistant/social prescribers, health and wellbeing coaches, care coordinators, occupational therapists, physiotherapists, physician associates, admiral (dementia) nurse, nurse associate, Advanced Nurse Practitioner, and a Community Matron.

Please see our staffing structure but note that it may be updated as we continue to develop and grow our team. Our staff sit within 5 teams: Pharmacy, Clinical, Health & Wellbeing, Operational and Frailty, with the aim of supporting our practices and improving the health outcomes for our patients.
In addition, we have built close working relationships with statutory and voluntary sector organisations including Linking Leeds, Primary Care Mental Health team, Memory Support Workers, LCH Neighbourhood teams, Adults & Health, local schools, Garforth NET and Rothwell Live at Home Scheme, libraries, Active Leeds, Local Care Partnership, Community Pharmacy and our care homes.
JOB SUMMARY We are looking for someone who is passionate about the role of the nursing associate and the opportunity to work with a multi-disciplinary team to deliver patient care. We have nurse associates in post, but this role is still quite new to our PCN, so we are looking for someone who is flexible, self-motivated, willing to embrace change and proactive in continuously improving service delivery.
The successful candidate will mainly be practice based, working across several practices with the other members of the primary care team including practice and PCN staff. Our nurse associates will work within the boundaries of their scope of practice and within the organisations policies and protocols, focusing on supporting patients with health needs, promoting self-care, monitoring long term conditions, health prevention and screening activities.
As well as providing care within our practices there will be an opportunity to support and deliver projects and services across our PCN. As part of this role you may be expected to work across various sites and settings within the community e.g. care homes, within a practice, or in a patients home.
In addition, there may be a requirement to provide leadership and mentorship to trainee Nurse Associates within the PCN, and you will work with our Lead PCN Nurse to support with the development, rollout and running of PCN-wide projects and services e.g. our Leg Clubs, spirometry and our diabetes work.

KEY RESPONSIBILITIES Practice Assist and perform routine tasks related to patient care as directed by senior nursing staff or GPs Following agreed clinical protocols with referral to senior nurses/GPs as appropriate assess, plan, develop, implement and evaluate programmes to promote health and wellbeing Provide nursing treatments to patients in participation with GPs or Senior Nurses to agreed protocols Able to document consultations according to NMC guidelines To carry out physical health interventions including vaccinations, childhood immunisations, cytology, venepuncture, ECGs, wound care and medicines administration commensurate with level of training received and with support provided from Senior Nurses as appropriate Support registered nurses to enable them to be able to focus on the more complex clinical care; Develop relationships across the PCN to support integration of the role across health and social care including primary care, secondary care, and mental health Promote health and well-being to all patients, for example undertaking the NHS health checks; Undertake assessments and reviews for individuals with dementia, mental health conditions, and learning disabilities Ability to makes judgements which require assessment of facts, e.g., escalating abnormal clinical observations, contraindications, emergency situations, wound care etc.

Administrative and Professional Responsibilities Recognise and work within own competence and professional code of conduct as regulated by the Nursing and Midwifery Council (NMC) Participate in the administrative and professional responsibilities of the practice and PCN teams Ensure accurate notes on all consultations and treatments recorded in patient SystmOne notes Attend and participate in practice and PCN meetings as appropriate Assist in the formulation of PCN philosophy, strategy and policy and develop appropriate protocols Managerial Contribute to the assessment of service needs Assist patients to identify their health needs Manage and organise individual patient consultations Aware of identification and reporting procedures relating to professional standards To contribute to and support the learning of e.g. apprentices, HCAs, future NA trainees and student nurses Work with the multi-disciplinary team Personal & professional development Maintain continued education by attendance at courses as deemed essential for professional development Participation in annual appraisal review Maintaining a record of own personal and/or professional development Governance Produce complete and accurate records of patient consultation, in line with best practice, confidentiality, policies and procedures Deliver care according to NHS guidance, NICE guidelines and evidence-based care Take part in the maintenance of quality governance systems and processes across the PCN and its activities.

Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required. Work with other clinical teams on improving the quality of healthcare in response to local and national policies and initiatives as appropriate Evaluate patients response to health care provision and the effectiveness of care Support and participate in shared learning across the PCN and wider organisation Awareness of statutory safeguarding, notification processes and local guidance for children/vulnerable patients, applying relevant policies and legislation to protect them Ensure compliance with policies, procedures and guidelines for self and others, by taking action or alerting senior management team if the practice appears to contravene policy, or if there are concerns over any aspect of patient care.

Health and Safety The post-holder will manage their own and others health & safety and infection control as defined in practices Health & Safety and Infection Control policies and published procedures. Comply with practices health and safety policies by following agreed safe working procedures Awareness and compliance with national standards of infection control, hygiene, regulatory / contractual / professional requirements, and good practice guidelines.
Correct personal use of Personal Protective Equipment (PPE) and ensuring correct use of PPE by others, advising on appropriate circumstances for use by clinicians, staff and patients. Reporting incidents using the organisations Incident Reporting System Confidentiality Maintain confidentiality of information, acting within the terms of the Data Protection Act and Caldicott guidance on patient confidentiality at all times.
