Typical day as our Support Coordinator

Creating person centred support plans with our customers, working with our clinicians to develop customers Risk Management Plans.
Sporting customers with independent living skills such as budgeting, maximising customers income, cooking, engaging with external services.
Promoting Home Group’s values within your team.
Work in close collaboration with; local authority, NHS mental health teams, safeguarding teams and housing option teams.

You bring
Passion to support customer to live their best life, working together with an eye for detail.
Experience of coordinating and signposting to internal and external services.
The ability to work on your own initiative, remain calm under pressure and have a resilient approach.
Recognise every customer is different and you’re able to adopt a flexible, personal approach, listening to customers to understand their needs.

Due to limits on our current certificates of sponsorship, we are not able to offer sponsorship to external & internal candidates for this role. This remains under regular review.

You’ll join Jodie Senior Client Service Manager and Natalie our Client Service Manager, we’re super passionate about what we do! We’re really proud of the service as it super unique to the area of Sunderland. We’re a supportive bunch and like to have cuppa’s and chats.

Job details

Where we’re flexible with you, we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion.
Working to a set rota pattern of day shifts, including weekends and Bank Holidays.
Able to use technology to update outcome spreadsheets, complete online learning and to collaborate with colleagues and our stakeholders within the City.

You’ll need an Enhanced DBS check done and we pay for that.
