Job overview

Are you a registered nurse who would like to work in the community. We are looking for a full time (37.5 hrs) or part time (30 hrs) RN to work in the beautiful market town of Malton within its rural farming community. You will join a well established community team comprising of clinical leads, DN, staff nurses and HCA as well as our our therapy team.

You will have a planned induction which includes a training programme delivered by our specialist nurses & for newly qualified staff we have a preceptorship programme. We have a clear competency framework so you know what is expected within your role & can plan development of your competencies with support from your line manager.

As a community nurse you will work closely with the wider MDT within our own services and other agencies including the acute hospitals, Hospice, General Practice, Social Services & the voluntary sector.

Main duties of the job

The main duties of this role will include -
  • Holistic patient assessment, formulation and reviewing of care plans
  • Holding a non-complex patient caseload
  • Management of pressure ulcers & wounds
  • Palliative & end of life care
  • Bladder & bowel care including catheterisation and continence assessments
  • Medicines Management
  • Injections (sub cut & IM)
  • Phlebotomy
  • Provision of equipment to support care
  • Safeguarding
  • Participation in daily safety huddles & handovers
  • Line management & supervision of junior staff
  • Practice supervision for nursing students
  • Continual learning and professional development

Person specification

Qualifications and Knowledge

Essential criteria
  • Part III or 13 of NMC live register
  • Degree/diploma in adult nursing
  • An understanding of relevant clinical practice/standards within identified clinical area
  • Standard IT skills
Desirable criteria
  • Evidence of current CPD/formal qualifications at an advanced level e.g. degree or post graduate level or equivalent knowledge/experiential learning
  • Full understanding/application of relevant clinical practice/standards/audit within identified clinical area

Experience

Essential criteria
  • Appropriate experience of working in Community Nursing
Desirable criteria
  • Evidence of practice skills and able to demonstrate the impact of this on practice change/development

Skills and Competencies

Essential criteria
  • Able to demonstrate effective interpersonal/communication skills Evidence of good time management skills
  • Ability to commute between the various sites
Desirable criteria
  • Working knowledge of Trust policies and procedures.
  • Ability to manage daily operations and maintain a safe working environment
