Fellow in Paediatric Cardiac Intensive Care & Mechanical Support ST6

The post will be based on the Paediatric Cardiothoracic Intensive Care Unit at Freeman Hospital with responsibility for outreach to paediatric cardiology patients on the Ward and 6 bedded HDU when on call. On PICU the post holder is responsible for a 12 bedded quaternary Paediatric Cardiac Intensive Care Unit.
The rota is worked as a full shift system with out of hours 2nd call responsibility for the Paediatric Cardiology Ward. Full consultant supervision and support is provided. Whilst the unit throughput is primarily cardiac with over 300 cases per year it delivers respiratory ECMO and has done so for more than 20 years.
It is one of the 2 UK Paediatric Cardiopulmonary Transplant Centres. There is also an active mechanical support programme for children with end stage heart failure. We have the second biggest Berlin Heart programme in the world and significant experience with other temporary and durable mechanical support devices.
There is an active research programme with specific interests in mechanical support as a bridge to transplant, infant respiratory failure and coagulation management. The post holder will be encouraged to carry out a research project / audit during their time in post and to present this at a national / international meeting.
The post holder will be responsible, in due course, for organising the departmental ECMO/mechanical support meetings and take an active role in the related teaching programme. On-call Commitments: The rota is worked as a 48-hour week full shift system.
It is a hybrid rota 9-person rota incorporating trainees from Paediatric Cardiac Anaesthesia (2), Paediatric Intensive Care (5) and Advanced Paediatric Critical Care Practitioners (2). A Paediatric Cardiology Registrar is 2nd on for Paediatric Cardiology with Resident SHO, so the role of the PICU trainee on the Paediatric Cardiology Ward/HDU is for outreach support and review of deteriorating patients.
Daytime work is based on PICU. Training and Research opportunities: In addition to clinical exposure to all aspects of Paediatric Cardiac Intensive Care Medicine and adopting a senior role when on clinical shifts, there are plenty of opportunities for research in the fields of mechanical support and heart failure.
These ventures will be actively supported by the department and will be factored into clinical working pattern. Successful applicants will be expected to take part in the departmental mechanical support and CHD research meetings. There is now a mobile ECMO service in the region run from our department with potential for trainees to become involved.
There are opportunities to attend theatre and echo clinics. Educational meetings comprise a full day every month of protected teaching in PCICU and additionally there is a programme of Cardiology and Cardiac Surgery teaching. Development of non-technical skills is supported by opportunities for the senior trainee to lead ward rounds, handover, communication with other teams and bed management.
Flexibility: In line with the Trusts core value of placing patients at the heart of everything we do, we are developing our service provision to be responsive to the needs of our patients. To meet those needs some staff groups will be increasingly asked to work a more flexible work pattern so that they can offer services in the evening or weekend.
