We have an exciting opportunity to join the Discharge Planning Team working as a Clinical Discharge Co-ordinator based at Bedford Hospital.

The Discharge Team facilitate and promote the safe, timely and efficient discharge of patients from the Trust.

The role will be ward based working collaboratively alongside the Multidisciplinary team to assess, agree and plan patient discharge pathways.

The post holder will have previous experience in Discharge Planning within a community or acute setting and will provide expert knowledge, advice and support for complex discharges.

The successful candidate will work collaboratively with external partners from community and adult social care.

You should be well organised and have excellent communication skills, being able to use your own judgement to problem solve to resolve any discharge delays.

The Discharge Planning Team covers a 7 day service working from 08.00 till 18.00 (including weekends and Bank Holidays).

Main duties of the job
  • Lead and oversee the co-ordination of adult patient discharges from the Trust
  • Ward based working as part of the Multidisciplinary Team
  • Actively participate in MDT Huddle and Board Rounds
  • To support ward teams in ensuring that all patients have a discharge plan developed within 24 hours of admission as appropriate
  • Ensure effective communication channels are maintained between the MDT in regards to patient discharge
  • Undertake patient assessment, collect necessary information, identify and allocate patient discharge pathways in collaboration with the MDT
  • To case manage and co-ordinate the discharge plans for patients requiring complex support
  • Work collaboratively with external partners from community and Adult social care to ensure discharges are safe and effective for the smooth transition into community care
  • Undertake mental capacity assessments as required
  • Undertake assessments for equipment ordering for discharges
  • To facilitate and complete Discharge Notification documentation for referral and allocation to community pathways
  • Facilitate and lead extended MDT meetings

  • Monitor and manage specialist discharge pathways including Rehab, Neuro and ABI pathways
  • Provide leadership and support to ward teams for the management of patient discharges
  • To Liaise and communicate with patients, families and carers when planning discharges
  • To encourage patients, families and cares to be actively involved in the discharge planning process
  • Facilitate nursing handovers to care facilities and agencies prior to patient discharge
  • Support ward areas with the Community Nurse referrals process

Qualifications

Essential criteria
  • • Registration to NMC or HCPC

Experience

Essential criteria
  • Previous experience of Discharge planning within an acute/community setting
  • Experience of managing a caseload of patients
Desirable criteria
  • • Experience in managing complex discharges from an acute or community setting

Knowledge

Essential criteria
  • • Knowledge of discharge planning processes
  • • In-depth knowledge of the health and social care national agenda plus legislative frameworks

Personal Skills

Essential criteria
  • • Be able to prioritise workload appropriately to meet tight deadlines
  • • Ability to perform effectively under pressure while delivering high quality outcomes within agreed timescales
