Are you an autonomous Nursing Associate who can provide patient-centred care in the patient's home? Or are you a newly qualified Nurse Associate that has a learning focus and a passion for community care?

We are a professional, skilled and friendly District Nursing team who work safely with our patients in the North East Derbyshire and Chesterfield area. We want to make a difference to our patients lives by providing evidence-based nursing care.

Main duties of the job

Provide high quality rapid nursing care to our patients in our local community to keep them safe at home, prevent hospital admission and support hospital discharge home.

Assess, plan, implement and evaluate individualised patient care in the most efficient and clinically effective way whilst maintaining the patient’s privacy and dignity.

Provide advice and support to junior staff in the delivery of high quality care and actively support the Community Nursing team in the management of the health care environment.

Implement/refer to the relevant services as appropriate.

Work closely with the Integrated Community Teams/GP surgeries and local acute Trusts.

  • Registered Nursing Associate on the NMC register/working towards
Desirable criteria
  • Community experience
  • Wound Care experience
  • Lower Limb training

Experience and Knowledge

  • Working in health and or social care environment
  • Understanding of factors that influence health and wellbeing
  • Following treatment plans
  • Risk management and safeguarding
  • MDT or integrated team working
Desirable criteria
  • SystmOne experience or other electronic patient documentation system
