Hospital Alcohol Liaison Worker - Chesterfield
Following securing additional funding, we have an exciting opportunity at our Derbyshire Recovery Partnership service for someone that is passionate about helping people to address their alcohol use and related harm. The role will involve working within the Chesterfield area of Derbyshire, conducting home visits within the community and outreach within Chesterfield Royal Hospital, working 37.5 hours per week Monday-Friday 9am-5pm.
The Role
We are looking for an alcohol practitioner to work closely with the Chesterfield Royal Hospital Liaison Team Nurses to provide support and treatment to people with alcohol issues that are accessing the hospital's emergency department and inpatient wards.You will engage, motivate and support people using alcohol into treatment and will deliver harm reduction and alcohol treatment interventions. You will assess the needs of people referred to you, develop care plans, complete risk assessments and development risk management plans and deliver 121 psychosocial interventions to appropriately safeguard the people on your caseload.
You will be working in a recovery-focussed way and will work closely with the service's Recovery Connector team as you support individuals to access local recovery activities and mutual aid groups.
About You
The ability to work independently, use your initiative, have good organisational and time management skills and work professionally with partner agencies, are essential for this role. You will receive lots of training and support in this role.Due to the outreach nature of this role you will need a driving licence with access to your own vehicle. Travel expenses can be claimed for and costs of parking eg at Chesterfield Royal Hospital will be reimbursed. Funding for this role is currently to 31/03/25, however there is possibility of extension to funding which may see this role become permanent.
