When you join Your Trust as an Exercise Referral Coordinator you will be part of a charity based in Rochdale that aims to support

Salary: Band 4, £27,975 - £31,641 per annum (pro-rate for part time)

Hours: 37 hours per week

What will my role be as an Exercise Referral Coordinator?

The purpose of the job is to provide a professional, efficient and high quality service for customers of Your Trust with long term
health conditions. This includes working with the wider Development and Leisure Teams to develop, implement and improve the offer
to customers, delivering as part of our Exercise Referral Scheme, undertaking health and lifestyle consultations with referral
clients, informing, educating and encouraging participants on how they can make changes to their lifestyles leading to long term

health improvements, ensuring knowledge of wider support available is maintained and managing confidential data.

What do I need to have?
  • Qualified to Level 3 or equivalent in a health, wellbeing or sports related subject
  • Hold Level 3 GP Referral qualification
  • Hold Level 4 Cardiac qualification or demonstrate commitment and ability to achieve within 12-18 months
  • Experience of developing and delivering health, wellbeing and fitness opportunities
  • Experience of working with people from diverse backgrounds
  • Ability to develop positive relationships with stakeholders, partners and members of the public
  • Ability to motivate and support people of all ages to lead active and healthy lifestyles
  • Friendly, outgoing person who likes to work with different clients
  • Ability to follow laid down procedures, work under pressure and without constant supervision
  • Understanding of how to support clients with medical conditions not yet under control
  • Excellent communication; both oral and written and excellent interpersonal skills
  • Excellent time management skills
