Main Duties and ResponsibilitiesTo deal courteously and efficiently with incoming telephone calls from healthcare professionals,patients and members of the public. Liaise, where necessary, with Dietitians or action as appropriateto roleOffice administration.

Provide cover for some secretarial duties during leaveAssist with the supply of therapeutic dietary products and equipment to the service.Check stock deliveries against delivery notes where required.Assist the Dietitians with monitoring of product supplier company performance against contract.Assist Dietitians with the arrangements for patients on tube feeds.This may include completing computerised registration for the home feed service, telephone calls andemails to GP surgeries, liaison with acute Dietitians and the home feed service company.

Enter dataas needed into tables..Provide follow-up support to selected patients who have been seen first by the Dietitian and monitornutritional care plan this may be in clinics, patients own homes or nursing homes.Gather information for referrals made to the DietitianVisit patients homes to undertake weight assessmentsActively participate within group education under the supervision of the DietitianAttend Trust mandatory training.Attend and participate in meetings as requiredProvide training on malnutrition universal screening tool/ nutritional screeningwhere required.Support nutritional analysis using computer software, where requiredAdhere to the Health and Care Professions Council (HCPC) and British Dietetic Association (BDA)4Responsibility for Policy and Service Development and ImplementationCommunicate to the Dietitians issues requiring further development.Assist in the development of patient advisory literature.Assist in quality assurance for care and delivery of service to the Trust.Propose changes in working practices or procedures for own area.Participate in quality improvement initiatives within the departmentResponsibilities for Human Resources and LeadershipTo work as an active, efficient and supportive member of the Nutrition and Dietetic Service staffacross several integrated teams.Responsibilities for Teaching and TrainingAssist in the development and delivery of education and training programmes.To support new/inexperienced staff members by participating in their induction programme/ trainingas required.Support the training of Dietitian students where appropriateResponsibilities for data and information resourcesAccurately record statistics on activity and audit in compliance with Trust and departmentrequirements if required.Complete nutritional analysis of recipes and menus as directed by senior DietitianResearch, Development and AuditParticipate in departmental audit as requiredParticipate in relevant policy development as requiredPhysical Skills and EffortManoeuvre goods and equipment and assist people to move, in accordance with manualhandling regulations and good practice.Standard keyboard skills requiredThe role requires physical effort to carry scales and hoist scales to homes and use with patients.This involves working within spaces in the patient homeAnthropometric measurements may need to be undertaken following instruction from the DietitianThe role will cover the region and requires movement between locationsGeneral Duties for all employees5Hand HygieneTo assist the Trust in reducing healthcare acquired infections (HCAIs) all staff should be familiar with allthe Trusts Infection Prevention policies which are appropriate to their role.

You are required to attendmandatory training in Infection Prevention and be compliant with all measures known to be effective inreducing HCAIsSafeguardingAll Stockport Foundation Trust employees are required to act in such a way that at all times safeguardsand promotes the health and well-being of children, young people and vulnerable adults.
Familiarisationwith and adherence to the policies and procedures relating to child protection and safeguarding ofchildren, young people and vulnerable adults are adhered to. It is an essential requirement of all employees asis participation in mandatory safeguarding training in accordance with the employees roles andresponsibilities.PreventionTo actively work with patients/clients/service users to identify appropriate opportunities in Making EveryContact Count for preventative interventions which may reduce the risk of future harm to health andwellbeing and to provide brief advice and refer or signpost to sources of further information and supportwhich may include advice on lifestyle behaviour and social care needs as well as safety andmanagement of long term conditions.Data Protection, Confidentiality and Information GovernanceThe post holder must abide by all relevant Trust and departmental policies including informationgovernance, confidentiality and data protection and, undertake the annual data security awarenessmandatory training.

The post holder is reminded that any breach of the Trusts information governanceand security policies and procedures, including data protection legislation, will result in disciplinaryaction.Data Protection Legislation the post holder is required to process all personal data relating to patientsand staff, whether in paper, electronic or other media, in accordance with the Data Protection Act 2018and the General Data Protection Regulations (GDPR), ensuring the security and confidentiality of data atall times.The post holder must not for their own benefit or gain, or to divulge to any persons, firm or otherorganisation whatsoever, any confidential information belonging to the Trust or relating to the Trustsaffairs or dealings which may come to their knowledge during employment.Health & SafetyUnder the Health and Safety at Work Act 1974, the Trust has a statutory responsibility to provide andmaintain a healthy and safe environment for its staff to work in.

Equally the post holder is required towork within the Trust Health & Safety Policy and fulfil a proactive role towards the management andcontrol of risk. This entails the identification, assessment and the immediate reporting, using the Trust6Incident Reporting System, any incident, accident, hazard or near miss involving patients, service users,carers, staff, contractors or members of the public.

If successful this vacancy will require an enhanced DBS check at a cost of £54.50. This cost will be deducted from your first Trust salary In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.
