To support the CHC Senior Management Team in the delivery of the statutory duty of NHS Continuing Healthcare (CHC) ensuring that the commissioning of packages of care, placements and / or services is clinically supported, that Service Specifications are in place and representative of the needs f the relevant population.
To support the CHC Team with systems to assure on quality-of-care provision, care co-ordination and care management, care and support planning, care pathway design, and commissioning and procurement of NHS Continuing Healthcare and NHS-funded Nursing Care, including services purchased with a Personal Health Budget To liaise as required between the various teams and services to ensure no gaps in the care pathway for adults and/or children in need for CHC services.
Main Duties and Responsibilities (approximately 15 - 20 areas) Responsible for the operational management, leadership and direction of the Continuing Care Team ensuring the capability and competency of both clinicians and clinical support staff.
Support the CHC Team Senior Managers to review the roles of clinical staff in the Continuing Care Team on an ongoing basis to ensure an effective skill mix and to enhance the productivity of the team. . Maintain up-to-date specialist knowledge on NHS Continuing Healthcare and associated legislation and will disseminate to the wider health and social care community as necessary, making the appropriate changes as required.
Ensure that all CHC assessments and reviews carried out by the CHC Team are of good quality through regular clinical audits. Ensure early identification of any performance issues with regards to clinical workforce and instigate appropriate supportive measures in line with ICB policy and guidance.
Maintain monthly overview of activity and spend within own service area and ensure early identification of increased activity and/or spend to Senior CHC Team. Be a clinical resource for staff, colleagues or the public to provide specialist advice, support and information etc.
in the handling of NHS Continuing Healthcare, related queries, appeals, retrospective reviews or local resolution panels etc Ensure that decisions, as a result of consideration for CHC, appeals etc. are communicated appropriately to the individual and/or their representatives in line with DoH guidelines.
Support with the Discharge to Assess and Transfer of Care functions Deliver training and support in the delivery of the National Framework to all partners Independent reviewing of cases for local resolution against decisions Support the CHC case management function Support to the Mental Health, Learning Disability CHC and Childrens Continuing Care Teams.
