Job overview
We are looking for two forward thinking individuals that hold a passion for bladder and bowel conditions that are highly motivated in treatment and management to join our enthusiastic North West Norfolk Team.
This position will mainly be working in the North and East side of Norfolk.
Job purpose- To assist in the delivery of high standards of continence care to patients within their own homes, care homes, the travelling community and in clinics, by following prescribed packages of care.
- To provide a high standard of care to the patient whilst promoting continence, managing incontinence, and promoting their independence and respecting their privacy and that of their families.
- Engage with mobile working

- Engage with referral to discharge standard processes

- To work within the continence service carrying out assigned tasks involving direct/indirect care in support of a qualified nurse.

Detailed job description and main responsibilities- Apply competency based treatment/care to patients with specific continence needs under the supervision of a qualified practitioner, following assessment and prescribed treatment/care plan.
- Plan and prioritise own visits to delegated patients, as highlighted on the designated rota template.
- Ensure patient held records are completed for each visit, and that SystmOne inputting is completed on a daily basis
- Make basic judgements on patients’ response to prescribed treatment/care plans, and report findings to a qualified practitioner.
- Liaise with other health care professionals as required.
- Overcome barriers to understanding, e.g. patients’ with hearing or cognitive impairment.
- Communicate with patients/carers by exchanging factual information, reassurance, tact and empathy.
- Establish and maintain appropriate working relationships with colleagues, patients, carers and other health care professionals.
- Be responsible for ensuring equipment is used safely, following relevant training, and that it is maintained appropriately.
- Contribute to the requisitioning of supplies and equipment through electronic ordering systems including stock checks for clinics, stores stock checks and checking expiry dates etc.
- Work predominantly independently on specified tasks, with regular clinical support and supervision by a qualified practitioner.
- Work with qualified practitioners with group work/clinics as required.
- Demonstrate an awareness and understanding of consent, and gain consent, and gain consent as appropriate following organisational policies.
- Share responsibility for maintaining store cupboards, pool cars, cleaning specialist equipment and general housekeeping tasks.
- Engage with operational hub, sharing responsibility of indirect patient contact tasks, such as, answering phones, arranging appointments, processing referrals and inputting activity data.
- Undertake health promotion/education as required, e.g. smoking cessation, falls etc.
- Will be exposed to bodily fluids, infected material, blood products on a daily basis, therefore must utilise universal precautions and adhere to infection control policies.
- Demonstrate basic IT and standard keyboard skills.

Essential criteria- NVQ 2 in health and social care, or equivalent knowledge and experience
- 2 GCSE’s Grade C or above in Maths and English or equivalent experience
- Experience of working with the general public
