The post holder will be responsible for a wide portfolio of work delivering against national ambitions and planning guidance at a local level, adopting a population health management approach and will include a quality improvement approach which focuses on reducing health inequalities.

As part of a dynamic team you will lead one or more programme area including Cardiovascular, Stroke, Diabetes, Respiratory and Prevention (Tobacco, Alcohol, Obesity, TB) for the BSol ICB. The post holder will provide senior manager responsibility and support for the development and delivery of the following within the Birmingham and Solihull (BSol) Integrated Care System (ICS): Innovation and transformation across the LTC and Prevention Programmes Reducing health inequalities across all workstreamsSupporting a BSol ICS shared vision LTC and Prevention programmes Improving outcomes and experience for individuals at risk of, or living with long term conditions so that they can live well, age well and stay well Change management and improvement science Programme and project management The post holder will also be responsible for; This postholder will primarily lead the Respiratory portfolio including Pulmonary Rehab, Home Oxygen Service, Community Diagnostics, Long Covid, LTBI, TB Service, and Clinical Pathways Development and Integration.

The post holder will work closely with other Senior Managers in the team to support wider LTC and Prevention programmes. Development and oversight for the delivery of the LTC and Prevention Programmes Responsible for Long Term Conditions and Prevention related policy, guidance and service development and implementation Directly managing the LTC and Prevention support team and being responsible for the day-to-day work assigned to the Team Line manager for the LTC and Prevention Project Managers and Support Officer The roles include responsibility for recruitment, career development and performance management Key relationships: You will proactively build good working relationships, including the provision of information and advice, to a wide range of internal and external stakeholders such as: BSol ICS LTC and Prevention Board LTC and Prevention Programme Boards including CVD, Stroke, Diabetes, Respiratory, End of Life, Frailty, and Prevention Neighbourhood Networks, Locality Hubs, Primary Care Networks and Providers ICS Communications and Engagement ICS leads and Place-based leads Secondary care providers, acute trust, community trust and private providers NHS health and social care staff within BSol ICS including primary, community and secondary care, LA, voluntary and third sector commissioners, providers, clinicians, managers and contractors, patients and their representatives for diabetes and related topics Local, Regional and National lead from NHS England, Local Authority Public Health, OHID, UK Health Security Agency (UKHSA), AHSN, DUK External consultancies working on other related projects and programmes within ICS Quality, Business Intelligence, Primary Care Commissioners, Finance, Contracting, Analysts.
