Primary Care Transformation Business Operations & Transformation Lead
Primary Care Strategy The post holder will provide the full range of project management capabilities which will include: Translate the strategy into programme and project plans, co-designing with clinicians and ICB staff to ensure clarity on where the responsibility sits for delivery Ensure and agree milestones and inter-dependencies ensuring the identification and agreement of timescales for delivery Allocate work plans to other members of the project team, ensuring oversight of delivery in line with milestones Ensure the development and implementation of effective governance and associated reporting Manage performance, ensuring the recording and escalation of issues, risks and delays to the external stake holders, including but not limited to GPs, HEE, Local Medical Committees and associate director setting out potential remedial action/solutions as appropriate Work with the Communications and Involvement team to ensure a strong patient voice, engaging with Healthwatch as required Ensure the delivery of the engagement and involvement approach, ensuring the active participation of a range of internal and external stakeholders which includes patients, patient groups and their representative bodies Horizon scanning to anticipate and ensure the alignment national policy with the local direction; identifying any challenges that may be faced that may compromise delivery Undertake regular surveys/audits to inform and review the delivery of the Primary Care Strategy and its associated programmes of work Reporting into the PC Transformation Board on a monthly basis on progress against key milestones.
Primary Care Support and Development Unit (PCSDU) The post holder will lead the development of the PCSDU; this will include: Work with clinical teams and senior team across the ICB to ensure the design of the PCSDU to ensure its functions align to the needs and requirements of providers To collect data and complete the necessary analysis to aid in the design and development of the unit To ensure the development of business cases; translating the vision of the PCSDU into viable delivery plans To work with external agencies (CQC and NHSE and other related regulators) to ensure their involvement in the design and delivery of the PCSDUs functions To ensure the active involvement of the collaborative in the development and oversight of the design and delivery Black Country Primary Care Collaborative (BCPCC) To work with members to design, develop and implement a roadmap for their development as the single voice of primary care providers across the Black Country Manage and co-ordinate the affairs of the Black Country Primary Care Collaborative (BCPCC) to work in line with agreed governance, showcase their work, engage with stakeholders, organise effective meetings and engagement events Lead and manage scoping and preparation of bids for funding on behalf of the Black Country Primary Care Collaborative (BCPCC) Plan and organise activities to align with transformation objectives working collaboratively with place-based teams, realise change providing expert knowledge and clarification where needed to achieve the objective(s).
Take budgetary responsibility for recurring ICB funding to Black Country Primary Care Collaborative (BCPCC). Be responsible for the financial transaction approval in support of the Associate Director of Primary Care (Transformation and Strategy).Determine, drive and support the development of the Black Country Primary Care Collaborative (BCPCC) identifying opportunities to showcase good practice and bid for funding that will aid transformation.
