Clinical Nurse/OT/Social Worker Lead post for Mental Health Integrated Network Team (MINT)

An exciting opportunity has arisen for a suitably qualified and experienced senior mental health practitioner to work closely with the Team Manager and other senior clinicians within H&F Central MINT services in Hammersmith & Fulham. This post is open to nurses, social workers and occupational therapists and is a management post within the team.

Remodelled community teams have been established over the past year following a successful bid for additional resources. In line with the population health model the team now deliver mental health interventions in partnership with primary care colleagues as well as continuing to provide care for those with more complex needs.

Background;

The Long-Term Plan, together with NHSE’s Community Mental Health Framework, provides a historic opportunity to address gaps in the provision of Mental Health care within communities. Community teams have moved away from siloed, hard-to-reach services towards joined-up care and whole population management approaches, establishing a revitalised purpose and identity for community mental health services.

One of the LTP’s key objectives is to develop “new and integrated models of primary and community mental health care [which] will support adults and older adults with severe mental illnesses”. It supports the development of place based and personalised community mental health services wrapped around the Primary Care Networks (PCNs), and outlines how these developments will help to improving care for people with severe mental illnesses.

Main duties of the job

The Senior Practitioners play critical roles in providing nursing leadership and MDT accountability within the Multi-disciplinary MINT teams. The teams offer enhanced access and provision to psychological therapies, improved physical health care, employment support, personalised and trauma-informed care, medicines management and support for self-harm and coexisting substance use.

In addition, the teams will work proactively to address social and racial disparities, using a bio-psycho-social approach, which includes dedicated LINK workers and Peer support.

Key to the success of the teams is the engagement and liaison with the local Primary Care Networks as well as the Local communities in supporting the delivery of this vision. This work is ongoing and is critical in using all available resources effectively and in a joined up manner.

Essential criteria

  • Degree/diploma level supplemented by specialist qualification, training, experience, courses to masters level equivalent relevant to the care group

Experience

Essential criteria
  • Experience of supervising staff and students
  • Able to manage a caseload of people with complex mental health needs
Desirable criteria
  • Awareness of Trust policies and service specification
  • Extensive post qualifying experience in relevant service area
  • Lived experience of mental health issues

Knowledge

Essential criteria
