An exciting opportunity has arisen for a Paediatric nurse to join the Childrens Diabetes Service. The focus of this role will be to work closely with our multidisciplinary team (MDT) to support a patient’s journey from Diagnosis to the adult diabetes service across ELHT.

The post holder will jointly manage a caseload of patient’s from the ages 0-19 years old whilst working alongside a highly skilled team of PDSN's Paediatric Consultants, Specialist Paediatric Doctors, Adult Diabetes Specialist Nurses and Specialist Dietitians.

Our diabetes team is an early adopter of technology and uses examples of all current diabetes technology available to co-ordinate patient care, whether this is flash glucose monitors, insulin pumps and CGM.

The ELHT integrated diabetes service provides complex care to patients with diabetes across ELHT, both within the acute and outpatient setting.

ELHT offer an excellent induction package and as well as clinical care delivery, you will have the opportunity to develop your skills in service development e.g. undertaking clinical audits.

Main duties of the job

Working across sites within ELHT NHS trust, you will be expected to deliver patients centred advice, support and care for patients between the ages of 0- 19 years old with diabetes within the secondary care service, their families and carers.

In line with National Service Framework (NSF) and the Nationals Institute for Health and Care Excellence (NICE) guidelines you will be expected to provide assessment of patients’ needs and advise on appropriate diabetes management. Additionally you will be expected to support patients and families through all stages of their diabetes care.

The care of patients between 16 and 19 years of age is jointly provided by the paediatric and adult team and you will be expected to work alongside both the adult and the paediatric team for these patients to provide a seamless transition of care in to the Young adult service at 19 years of age at which point paediatric input will cease.

Your role will involve empowering patients with the skills and knowledge necessary to attain independence in self-management.

You will be responsible for maintaining and updating your own knowledge and skills, together with providing evidence in support of this.

Detailed job description and main responsibilities
  • To become a member of the multidisciplinary team and aid in the development of the Paediatric Diabetes Nursing Service in order to facilitate the early discharge and prevent avoidable re-admission to hospital for children with diabetes, through the provision of specialist support.
  • Facilitate the seamless transition from hospital to home through multi-professional/multi-agency working, across in-patient and primary health care services and other community settings, e.g. schools, nurseries and recreational organizations.
  • Enable children and young people with diabetes to reach their optimum level of health, independence, social and psychological/emotional well-being, through provision of continuous clinical, social and psychological/emotional support for the patient, their families and carers.
  • Carry overall continuing responsibility for the management of patient case-loads, promoting and maintaining high standards of nursing care.
  • Co-ordinate and delegate team work.
  • Provide clinical advice and support, education and training to healthcare professionals and others.
  • Support the Education Lead in planning, preparing and delivering Diabetes Education to various client groups in a variety of settings, both community and hospital.
  • Build strong links with schools and support teaching staff managing the specific health needs of children/young people with Diabetes
  • Support families both at home and in the community, in complying with their child’s diabetes regime.
  • Provide targeted support for children and young people with a high Hba1c.,
  • Liaise with other agencies.eg Surestart, Social Care etc. in providing the above support.
  • Overall continuing responsibility for the clinical assessment and autonomous decision-making with regard to individual care needs, within hospital and community settings.
  • Co-ordinate and delegate, as appropriate, nursing activities to other members of the Children’s Diabetes Nursing team.
  • Monitor the management and delivery of care to children and their families by members of the paediatric Diabetes Nursing Team.
  • Act in partnership with the child and family to continuously assess a child’s health, emotional and social needs. Plan, implement and evaluate care in accordance with the Directorate’s Philosophy of Care.
  • Manage and resolve differences between families, healthcare professionals and other carers regarding any issues, acting as the child’s advocate at all times.
  • Provide specialist support and education for children, families and other carers, at diagnosis and thereafter, to enable them to become confident in their abilities to meet the child’s health needs at home and in other community settings, e.g. in school, during recreational activities such as brownies, guides etc. and respite care.
  • Provide a specialised service for continuous insulin pump patients and their families within ELHT and those referred from outside the Trust. (Training can be arranged for post holder if necessary).
  • Provide a specialised service for continuous glucose monitoring (CGM) for patients and their families within ELHT (training can be arranged for the post holder if necessary).
  • Input into local support groups and their activities as required, to include some evenings.
  • Provide specialist nursing and emotional support for children, families and other carers during distressing times of a child’ life e.g. at diagnosis or when their condition deteriorates for whatever reason.
  • Provide ongoing information, advice and counselling for children, young people and their families concerning long-term health out-comes, including complications and lifestyle issues, such as job choices, driving and risk taking behavior, working with Clinical Psychology staff when indicated.
  • Act as an expert resource for children, families and other carers, such as teachers, recreational activity leaders and respite carers. Visit homes, schools and other public venues as required.
  • Provide complex technical and theoretical information in appropriate manner to facilitate the understanding of individual children and their families and other carers, to enable that understanding to be put into practice.
  • Motivate and empower children and their families and other carers to manage this condition.
  • Refer to/work with other specialist services/agencies as indicated or as necessary e.g. Social Services, Interpreter. Attend inter-agency meetings and review meetings when required.
  • Order relevant laboratory investigations and diagnostic procedures, interpreting the results, and communicating with other health professionals as indicated.
  • Oversee the organisation and running of diabetes clinics. Contribute to diabetes clinics and be prepared to run nurse led clinics when required.
  • Facilitate and support the provision of transition services for young people between paediatric and adult service providers.
  • Act as a prime clinical resource and support person for other members of the diabetes team, and other professionals in the paediatric in-patient hospital team and primary care team members.

Essential criteria
  • Registered as First Level Nurse (part 8 or 15)UKCC

  • Diabetes Nursing Qualification.

  • Educated to Degree Level or equivalent qualification.

Experience

Essential criteria
  • Experience of working with and managing children with diabetes
  • Significant post registration experience.
Desirable criteria
  • Experience of working in a diabetes related environment.
  • Experience of working in the community

Knowledge and Skills

Essential criteria
  • Awareness of the Paediatric Best Practice Tariff and its implications for care provision.
Desirable criteria
  • Good knowledge of clinical information systems.
  • Willingness to undertake insulin pump and CGM training.
