We are excited to offer a post for a specialty doctor to join our perinatal team and believe it is a great opportunity for their career development.

The Perinatal Team Community Team focuses on early diagnosis and early intervention, aiming to tackle mental health difficulties in the perinatal period at an early stage, thereby reducing and preventing life adversity and transgenerational trauma.

The perinatal team is a multidisciplinary community team with professionals from a variety of backgrounds. By joining the team, the specialty doctor will have the opportunity to develop their leadership skills, and to influence and inspire the perinatal mental health team.

They will contribute to the cohesiveness of the team, staff satisfaction and retention rates, and improvement in the clinical delivery of care. Furthermore, as perinatal mental health disorders often require a prompt multidisciplinary intervention, this post will further develop the doctor’s skills to manage complex clinical /medicolegal situations and to network with professionals from various settings.

Main duties of the job

The post is in Peterborough City Health Clinic and with satellite clinics in Huntingdon (Newtown Centre) and Wisbech (Agenoria House). The speciality doctor will work with and under the supervision of the consultant psychiatrists together to provide clinical assessment and management for the perinatal patients with severe and complex mental health conditions in the area.
Along with the consultants the speciality doctor will also provide medical leadership to the North Perinatal Mental Health Team and offer advice, support and supervision to the MDT team. The speciality doctor and consultants are expected to form close working relationships with local maternity, health visiting mental health champions as well as primary care colleagues and the Primary care Mental Health team.

The speciality doctor and consultants are expected to foster close working relationship with the local Mother and Baby Units.

Detailed job description and main responsibilities

Key Clinical Responsibilities

The post holder will be working with the Consultant-led multi-disciplinary team as part of the Perinatal Mental Health Service. It is expected that the Speciality doctor is flexible to provide service within the team and take up senior roles supporting the consultant and junior colleagues within the team as and when required.

The various duties of the specialty doctor include:

  1. Provide high quality medical care with support from consultant perinatal psychiatrists across the Perinatal Service.
  2. Provide medical input to the Perinatal community Services. This includes carrying out initial assessments and psychiatric review of women’s mental health and mother infant relationships and related risks. This assessment should include a bio-psycho-social diagnostic formulation and care planning. Care planning includes decision making on prevention, treatment, investigations and care options. Treatment pathways should include therapy, care coordination, and pharmacology options, depending on the need. The speciality doctor is expected to maintain clear communication and shared decision making with patients (and families as appropriate), communication and joint planning with other professionals and agencies by the most effective means, use of appropriate legal statutes and powers where necessary for good patient care.
  3. Attendance at MDT meetings, as well as other patient reviews, prebirth planning meetings and other professional meeting.
  4. For the clinical work it is expected that system one notes will be kept including letter to the GPs and other professionals, discharge summaries, social care reports and Tribunal reports when needed, and other reports to external bodies are written and prepared promptly when requested.
  5. Under the supervision of the consultants, provide regular advice to primary mental health services.
  6. Support the Perinatal Consultant Psychiatrists in providing clinical supervision to junior staff that are working or training in the service, and offering clinical advice and information as and when required to all staff.
  7. Close liaison with the locality team, Crisis Home Treatment team, early intervention for psychosis team, psychiatry liaison team, health visitors, midwifes and IAPT.
  8. Contribute to the assessment of risk, and management plans to mitigate risk factors of women in the perinatal period. This includes joining the daily handover meeting.
  9. Maintain the high level of expertise and skill necessary for the provision of excellent quality perinatal mental health care, aiming for optimal outcomes for mother, infant and the wider family, along with the .
  10. Ensure that care provided is in line with vision and values of the service for patient centred, compassionate, high quality care focussed on the needs of the woman, the infant and the wider family, facilitating shared decision making at all times.
  11. Ensure highest standards of transparency and clarity in communication with patients and other professionals (e.g. all correspondence addressed to the patient and copied to all professionals).
  12. Work closely with all professionals in the Service and with other agencies, including voluntary organisations to ensure that high quality patient care and aftercare is effectively coordinated.
  13. Contribute to quality improvement activities in the Service. Expectation of involvement in the Clinical Governance programme and encouraged to participate in medical audit, and support the service to undergo expansion in line with NHS England Longterm Plan. These include RCPsych CCQI Perinatal Network Peer reviews and related surveys, CCQI visits, and reports to commissioners.
  14. Ensure that cover is arranged for responsibilities during leave with consultant colleagues

Additional Duties:

  1. Participate in annual appraisal and revalidation for speciality doctors.
  2. Participate annually in a job plan review which will include consultation with the Lead Clinician and Service Manager in order to ensure that the post is developed to take into account any changes in service configuration or delivery.
  3. The speciality doctor is expected to participate in teaching and training of other professionals including teaching of medical students and postgraduate doctors in training and out of training.
  4. The speciality doctor is expected to take part in regular mandatory training, and participate in ongoing CPD programs, whether internal sessions organised by the trust, university of Cambridge or external sessions from the Royal College of psychiatry or other organisations.
  5. Undertake the administrative duties associated with the care of patients.
  6. Ensure that clinical activity is accurately and comprehensively recorded.
  7. Part of the professional meetings, meetings and assessments are currently carried out virtually via Microsoft teams. The specialty doctor is expected to participate in both face-to-face and virtual encounters.

Education/Qualifications

Essential criteria
  • Full GMC registration and licence to practice
  • MBBS or equivalent
  • Eligible for approval under Section 12 of the MHA 1983
Desirable criteria
  • MRCPsych or equivalent

Entry Criteria for the grade

Essential criteria
  • Completed minimum of 4 years full-time postgraduate training (or equivalent) of which 2 will be ina specialty training programme in psychiatry or as a fixed term specialty trainee in a relevant specialty or shall have equivalent experience and competencies

Experience

Essential criteria
  • Proven experience of good working relationships
  • Experience of multi-disciplinary working

Skills/Knowledge/Ability

Essential criteria
  • Knowledge of policy and legislation relating to mental health
  • Ability to make decisions and carry out responsibilties without direct supervision
  • Ability to take clinical lead under the supervision of the Consultant
