The Tackling Diabetes Together Programme is committed to halting the rise in Type 2 diabetes and improving outcomes for people living with Diabetes. We are collaboratively working with colleagues from across NHS Wales, including: Health Boards, NHS Trusts, Public Health Wales, the Welsh Value in Health Centre, and the clinical diabetes network, third-sector partners and people living with diabetes to support and drive the programme.
Main duties of the job
As the Transformation Director, you will be at the forefront of driving strategic delivery and system-wide change.
Reporting to the National Director of Health and Wellbeing, you will lead the programme’s strategic initiatives, ensuring collaboration across partner agencies and workstreams to achieve our ambitious goals.
Key Responsibilities:
Programme Leadership: Lead the Tackling Diabetes Together programme, formulating long-term strategic plans and building an effective team to deliver objectives.
Implementation and Delivery: Oversee the implementation of workstreams, establish performance systems, and foster a culture of partnership and system leadership.
Communication: Clearly articulate the benefits of diabetes transformation, delivering complex presentations and ensuring regular updates on progress.System Leadership: Identify and address barriers to implementation, promote effective partnerships, and ensure strong clinical leadership.
Innovation and Improvement: Champion innovative solutions, including service redesign and digital health, to improve diabetes outcomes.
Governance and Stewardship: Ensure robust governance, performance management, and budget oversight for the programme.
Collaborative working: working collaboratively with a range of partners to help deliver the programmes outcomes

Qualifications and Knowledge
Essential criteria- Masters Degree OR relevant professional qualification OR equivalent experience in a relevant field such as healthcare, public health, management, or leadership.
- Strong knowledge of the healthcare system in Wales, including the role of NHS Wales and Public Health Wales, Primary care, National and local government
- Knowledge of methods of developing clinical quality assurance and evidence-based clinical practice
- Understanding of and commitment to delivery of improved health through mainstream NHS activities
- Significant demonstrable track record of strategic planning and development, working across multiple organisations and teams to deliver strategic outcomes.
- Understanding of Welsh Government and organisations principles and standards of governance, policies, strategies, and plans
- Evidence of and a demonstrated commitment to continued professional development
- Qualification in quality, systems leadership, clinical leadership or programme management
- Knowledge of digital health innovation and its role in public health improvement.
Experience
Essential criteria- Proven track record of successfully leading large-scale, complex transformation programmes within healthcare or a related sector.
- Experience of team leadership, programme management and performance management, with the ability to inspire, motivate, and develop staff.
- Demonstrate experience of building, maintaining and utilising successful relationships with all staff including senior management and clinicians within complex organisations. To influence at all levels and be a highly strategic thinker.
- Demonstrated success in managing stakeholders, including senior leaders, clinicians, and external partners.
- Experience of performance management, governance, and budget oversight in large programmes
- Demonstrable experience of influencing without authority
Skills and Attributes
Essential criteria- Excellent communication skills, with the ability to engage, build relationships and influence stakeholders at all levels, including political, corporate, and clinical.
- Ability to develop and maintain effective partnerships across the healthcare system.
- Strategic thinking with the ability to identify opportunities, resolve complex issues, and drive change.
- Highly developed analytical and problem-solving skills with the ability to interpret data and inform decision-making.
- Proven ability to achieve targets and objectives within a demanding and pressured environment against challenging deadlines, demonstrating excellent organisational and time management skills to meet competing priorities.
- Able to demonstrate sound financial management skills.
- Ability to demonstrate understanding and application of our workplace values, together with the underpinning behaviours identified for success in this role
- Self-motivated with the ability to work on own initiative as well as committed to empowering team members to work effectively
- Ability to champion innovation, particularly in service redesign and digital health.
- Welsh Language Skills are desirable
The salary scale above has been agreed as part of the NHS Agenda for Change pay award for 2024/2025 and will be implemented in November 2024 backdated to 1st April 2024 where applicable.

The salary scale shown is for a full-time member of staff, with the amount being adjusted pro rata for those working less than 37.5 hours per week.
