The post holder will support the Programme Director for Elective Care and wider colleagues across the Provider Federation Board and GM Integrated Care to successfully deliver the agreed work programmes. The post holder will work closely with clinical and managerial colleagues across the healthcare community to principally support transformation programmes, with the development and implementation of projects andservice changes to ensure that services are responsive to and meet the needs of the population of Greater Manchester.

This will involve engagement on service development, scoping national and local priorities,formulation of specific and appropriate transformation intentions, and supporting the delivery of a programme of work to include outcome-based service specifications for all services and robust contractual performance monitoring and management of services.
Partnership working is essential across the healthcare, social care and volunteer community as we continue to break down barriers between organisations and reshape the way we use resources.The post holder will collaborate with clinical and managerial colleagues to ensure care pathways support effective flow of patients between primary, community, intermediate and secondary care.
Managing complex agendas, influencing partners and negotiating sometimes complex and conflicting relationships. Through strong leadership, transformational approaches and change management skills the post holder will develop, implement, and evaluate innovative models of care delivery that meets the needs of service users and commissioners alike.
A primary focus will be to ensure improvements in patient / client outcomes and thesafety, effectiveness, and experience of their care. Provide identification of transformation projects, development of project plans, milestones and risks and the delivery of efficiency to enable service transformation.
The post holder will work closely with the Programme Director for Elective Care within their portfolio area and Clinical Leads to deliver the priorities identified within their programme of work. The post holder will be responsible for supporting the Operational Delivery plan for Elective Care which deliver high standards of care and which demonstrates achievement through quality and outcome indicators and drives efficiencies.
In collaboration with Communication Leads responsible for developing adequate links between clinicians, health and social care professionals, public health and local authorities including the Community Voluntary Sector, ensuring that all are kept well informed and can influence changes.

Present complex information about projects, initiatives and services to a wide range of stakeholders in a formal setting. Commit to working and engaging constructively with internal and external stakeholders on a range of contentious issues. Nurture key relationships and maintain networks internally and externally, including national networks.
Assist with public relations and marketing activities. Assist with Freedom of Information requests; keep under review the framework and in accordance with the legislation Functional Responsibilities Support and contribute to the Elective Care Programme priorities.
Oversee the implementation and development of Elective Care workstream strategy. Manage and contribute on leading on specific programmes of work. Manage and contribute to the lead specific transformation projects. Oversee coordination of tactical groups, Clinical Reference groups and sub-groups as required.

Key Responsibilities Monitor performance against key national standards, seeking assurance andescalating issues where required. Be part of the new contracting approach. Establish and manage specific service and pathway improvement initiatives todeliver locality and national planning requirements for Care pathways (including accelerated pathways and new models of care) Working with key partners across primary and secondary care and the voluntary sector to deliver the GM objectives Represent the Elective Care program at GM and national forums as required Project Management Responsible for the day to day delivery of a range of redesign projects, across the Elective Care Programme To ensure plans are robust and sufficiently detailed to provide assurance that delivery will result in the service improvements / developments and financial benefits.

Analyse complex data sets to enable the dissemination of performance information relating to demand management and be able to respond to all queries in relation to these. Interpret guidance to ensure new service models, service improvements and development of projects are in line with national agendas and the Elective Transformation Plan.
Work with Lead Clinicians, to ensure full clinical engagement, and professional ownership and implementation of service transformation plans. In collaboration with Lead clinicians, lead regular meetings for work-stream leads and seek opportunities for new business so as to maximise advantages for patients and services.
Work with other members of the team, providers and wider system partners to deliver the elective care objectives. Promote integration between professions and agencies and ensure services are co-ordinated across a whole system, reshaping and modernising care to improve patient outcomes and experience.
Support procurement of scoping work for transformation services, and promoting resilience and translation of outcomes into plans. Ensure meaningful patient and public involvement and the use of patient and community feedback in the commissioning development, planning and improvement of service delivery.
To operate in a political and sensitive environment. Operational and General Providing knowledge when supporting the procurement of scoping work on health services and resilience and translate outcomes into plans. Working internally and externally to deliver projects, initiatives and services to time and in a cost effective way.

To support the identification and sharing of best practice in all engagement. Support the portfolios of programmes in demonstrating value for money for the current spend through tracking, managing and delivering agreed benefits. Will be required to deliver challenging information to stakeholders across the Health and Social Care Economy Work with PFB and GM Integrated Care to evolve and develop predictive / early warning clinical / social quality dashboards that enable targeted intervention.

To report immediately any incidents, accidents, complaints or otheroccurrences involving patients, visitors or staff, resolve wherever possible, complete accurate statements and report to the Line Manager as soon as possible. Provide financial, activity, quality and other reports to the appropriate committees and boards, as part of the revised governance. Constantly strive for value for money and greater efficiency in the use of these budgets and to ensure that they operate in recurrent financial balance year on year.
Information Management Highlight exceptions and risks ensuring mitigating action can be taken to keep the programme on track. Drafting detailed reports summarising status on issues, appraising outcomes, and providing progress reports for the Programme Director and Programme Boards.
Collate as required, qualitative and quantitative information and lead appropriate analysis to develop robust business cases and contribute to projects. Analyse, interpret and present data to highlight issues, risks and supportdecision making.

Prepare relevant reports and papers in relation to the relevant service areas within the portfolio as required. Prepare responses to the NHS England, Department of Health or other organisations / individuals on behalf of the Elective Care System Board.
