Rehabilitation Assistant

The Rehabilitation Assistant / post holder is responsible for the provision of high quality, individualised personal care and rehabilitation programmes to our patients on Caterham Dene Ward, reflecting the philosophy of First Community Health and Care.
They will work under indirect supervision of the lead clinician and will manage their own upskilling caseload and be responsible for maintaining up to date patient records. They will carry out delegated patient related activities, utilising knowledge and skills, within assessed competency frameworks and will work collaboratively within the team to meet the needs of the patients, following policy and procedure.
They will enable the smooth running of the Bed Based Care Service through completion of day to day duties and delegated responsibilities. The post holder will be personally responsible for not undertaking any action which would knowingly cause risk to them, others or the company.
The team provides a seven day a week rehabilitation service at Caterham Dene Hospital and visit patients own homes to enable safe discharge. The Post Holder must have a car for personal business use and be able and prepared to drive between our Base at Caterham Dene Hospital and other community settings, as required, during their working day.
