- PCN Operations Care Coordinator Reports To : Emma Bullock PCN Manager Role Description : Chapeltown Primary Care Network (PCN) consists of 3 GP practices, Allerton and Westfield, St Martins Medical Centre and Woodhouse Medical Practice, with a population size of approximately 27,000 patients. The Care Coordinator role is to support the effective running of the PCN alongside the PCN Manager, whilst projecting a positive and supportive image. Primary Responsibilities : Provide general assistance, administration and operational support to the PCN Manager and Clinical Director. Create, run and analyse report data from clinical systems (S1 and EMIS) and within MS Office applications (Word, Excel and Outlook) and Ardens.
- calendar invites, agendas, presentation packs, compilation and distribution of meeting minutes and ensure actions completed in a timely fashion.
Create and manage small projects, defined by or in conjunction with the PCN Manager, to deliver desired outcomes in accordance with population health data for our patients.
For example :- Covid and Flu Vaccination Programme
- Cancer Screening
- NHS Health Checks Management of intranet (Teamnet) and PCN website Management of appointment rotas.
Secondary Responsibilities : Day to day patient interaction both in practice and in patients own homes.
Follow, support and comply with practice procedures, IT, infrastructure and telecom systems, and work in accordance with written protocols and procedures.Contribute toward the team ethic and support patients in line with the PCN principles and values.
