Job overview

A rare opportunity for a full Time community care coordinator in the Older peoples community mental health services. Based at Meridian House, you will join the AWCMHTe, covering the beautiful Aire and Wharfe Valleys.

Once inducted and confident you will have opportunity to work in an agile manner and from home on occassion .

Main duties of the job
  • Act as care co-ordinator for a designated group of clients in accordance with national and local guidelines.
  • Construct, implement, and evaluate packages of care to effectively meet the needs of clients and their carers.
  • Undertake accurate, timely, and skilled clinical assessments, and ongoing reassessments, taking into consideration all social, physical, and environmental factors.
  • Supports clients and carers in their understanding of a diagnosis and its implications for the future.
  • Maintains relationships with wards, departments, and all other agencies to initiate and facilitate excellent service provision
  • To help identify junior staff development and training needs to enhance service provision.

Qualifications

Essential criteria
  • RN (part 13) RMN Or HCPC registered OT or Physio or equivalent Or Social Worker
Desirable criteria
  • ENB 998 / Facilitating learning in practice
  • Counselling skills qualification
  • Clinical supervisor training
  • Computer literate

Experience

Essential criteria
  • Minimum of twelve months at Band 5 or equivalent responsibility in mental health
Desirable criteria
  • Experience of working with clients aged 65 years and over who have mental health needs.
  • Experience of being clinical adviser to junior staff or students.

Skills

Essential criteria
  • Communication – written/verbal
  • Articulate – meaningful, legible
  • Decision making
  • Use of initiative
  • Competent clinical skills
  • Assessment, planning, implementation and evaluation of care.
Desirable criteria
  • Language fluency, particularly South Asian (Punjabi, Urdu, Bengali, Hindi, Gujerati) or Eastern European (Polish, Ukranian)
  • Experience in Group work
