Main duties of the job

The Continuing Care Case Manger will be expected to work within the Continuing Care team to ensure that identified complex cases are managed appropriately using expert knowledge and autonomous decision making skills.

Working in partnership with patients, carers, GPs, case agencies, consultants, other health professionals and social care as appropriate, you will be expected to assess and review complex care packages to ensure they continue to meet on-going clinical needs.

You will lead and participate in Multidisciplinary Team meetings to ensure adequate outcomes of patient care packages and provide specialist clinical advice to the Triage panel for all patients accessing the Continuing Care service.

The role will include case management of adults assessed eligible for Continuing Care.

You will engage in clinical supervision and provide professional support and advice to colleagues and senior managers with the organisation.

Detailed job description and main responsibilities
  • The post holder will be responsible for the active management of a case load of patients with complex Continuing Healthcare needs that are either wholly or majority funded by the ICB. The post holder will support and advise on the commissioning of high quality individual packages of care, and work in partnership with the Local Authority Commissioners for care packages that are jointly funded. The Complex Care Case manager will support the development of and oversee the implementation of care plans for individual care packages. Providing regular updates to Head of Continuing Care.
  • Manage High cost complex care packages to reduce unnecessary expenditure where evident ensuring that care delivery is appropriate and proportionate to the identified needs of the individual ensuring the best use of resources
  • The post holder will offer clinical advice around complex packages of care and be an integral part of monitoring the quality of service provision.
  • The post holder will ensure that they and the team adhere at all times to the relevant National Framework and guidance, NMC Code of Professional Conduct and work within established policies, procedures, protocols and systems ensuring quality standards and performance
  • Make referrals to various services and coordinate the different referrals ensuring that referrals have been picked up an acted upon.
  • Ensure that care settings are of a high standard of quality linking into partner organisations to triangulate information where quality issues have been identified.
  • Report all safeguarding concerns to the appropriate organisations and support investigations as required.
  • Advise the ICB on future/ potential development areas, gaps in service or risks.
  • Be aware of national requirements around MCA and MHA ensuring that care plans and service providers meet the requirements within these legal frameworks.
  • Monitor and evaluate placement providers, reporting any issues to the wider ICB and other relevant professionals and organisations
  • The post holder will support the Lead Nurse to ensure that their workstream is planned and managed effectively and take the lead in assisting in their successful delivery.

Essential criteria
  • Registered Nursing Qualification and current registration with NMC
  • Educated to degree level in relevant subject or equivalent level qualification or significant experience of working at a similar level in specialist area, ideally CHC
  • Able to demonstrate knowledge base around MCA and consent issues
  • Knowledge of safeguarding process and responsibilities
  • Demonstrated experience in a Healthcare environment
  • Effective communication skills
  • Ability to analyse very complex issues where material is conflicting and drawn from multiple sources.
