This is a key role in supporting our Primary Care Network (PCN) teams to successfully implement transformation projects and initiatives across the organisation and provide high quality information for performance management and decision-making purposes.
The post holder will support the PCN management team as follows: Support the delivery of projects and initiatives which achieve the expected benefits on time and within budget. Take responsibility for managing specific workstreams within projects.
Coordinate the collation and submission of key data and information submissions. Provide high-quality clinical and non-clinical data and information to demonstrate positive change and inform the provision of healthcare. Develop and report on progress against key performance indicators and measures.
Monitors contractual compliance and performance. The Role will be required to work with a variety of data sets to create meaningful information and reports, working in partnership with central, practice and PCN teams. DUTIES & AREAS OF RESPONSIBILITY Key Responsibilities Project & Information Work with stakeholders on project initiation / proof of concept and defining key workstream criteria e.g., objective(s), scope, benefits, stakeholders, implementation plans etc.
Coordinate and support meetings as relevant. Complete and maintain relevant workstream documentation e.g., project plans, risk registers, communication plans etc. Monitor progress and proactively identify any issues and risks to successful delivery.
Support effective communication to internal and external stakeholders. Producing updates for internal and external use Analytical and Judgement Carry out timely and accurate information analysis. Report findings and the outcomes of information analysis in a variety of formats as required by the audience.
Extract data and reports from practice systems for local and national requirements. Support the design of, and run searches, in various clinical systems. Support the compliance, clinical and management teams to identify areas for improvement in data quality.
Benchmark practice performance within the organisation and outside the organisation. Manage the contract governance process and monitor contractual service levels and performance. Communications and Key Working Relationships Work in conjunction with central, site and PCN teams to support the delivery of transformation activities / initiatives and report and monitor performance against key actions and performance indicators.

Work in conjunction with site teams to monitor compliance with local and national enhanced services, vaccination & recall programs Communicate with a wide range of internal and external stakeholders to gather and share information Participate in relevant internal and external working groups to provide relevant information / analyst advice and support, and to maintain data collection / sharing systems for its effective use by the team Support PCN member practices with implementation of new digital and transformation initiatives.

Knowledge and Skills Ability to organise and maintain your own workload, working with minimal supervision for daily tasks. Ability to coordinate other staff for the purposes of project implementation. Ability to deal with working in a changing and customer responsive environment Support implementation of initiatives through timely and review information analysis and administrative support.
Experience of service changes and implementation of new systems Friendly, professional, and approachable Highly motivated and reliable. Able to work under pressure. Articulate and numerate. Good working knowledge of MS Office, including Excel, Outlook, and Word.
Experience of working with SystmOne and designing and running searches Other Be willing to undertake ad hoc travel to various locations to carry out duties of the post as required. To safeguard the health, well-being, and safety of the patients we work with, some of whom maybe classed as vulnerable people or adults at risk.
In the event of a risk to a Patient becoming apparent or if concerns arise about a vulnerable persons welfare, to immediately report these concerns in line with the appropriate policy and procedure NB. in addition to these responsibilities, employees are required to carry out such other duties as may be reasonably required.
