The role of the ARTU co-ordinator is to manage referrals and complex discharges for patients in specialised rehabilitation beds on the unit.

Main duties of the job

The successful applicant will be working alongside a large MDT team coordinating all complex discharges, including individual funding request submissions and liaising with CCGs, complex care teams and social care teams across Staffordshire, Cheshire, Shropshire, Telford and North Wales.

Another aspect of the job role is supporting the coordination of

MDT meetings, ensuring that all relevant data is collected and made available to the appropriate team members. Along with supporting the clinical team in arranging and communicating individualised Goal Planning Meetings for all patients, liaising with families/carers frequently.

Detailed job description and main responsibilities
  • To work as a pivotal member of the ARTU team, to co-ordinate the referrals and complex discharges for patients in specialised rehabilitation beds on the unit.
  • To facilitate and coordinate the efficient and timely admission to and discharge from the specialised beds with the input from the appropriate team members.
  • To assist in the delivery of a first class service and to work closely with the clinical team, providing accurate and relevant information to designated recipients.
  • To support the clinical team by coordinating all complex discharges, including individual funding request submissions and liaising with CCGs, complex care teams and social care teams across Staffordshire, Cheshire, Shropshire, Telford and North Wales.
  • To support the coordination of efficient and effective MDT meetings, ensuring that all relevant data is collected and made available to the appropriate team members.
  • To support the clinical team in arranging and communicating individualised Goal Planning Meetings for all patients, liaising with families/carers frequently.
  • To collect and submit data on all activity as per NHSE/I requirements. Identify areas for improvement to streamline and improve the quality of the patient pathway and to improve processes across the service.
  • To support the UKROC (UK Rehabilitation Outcomes Collaborative) process in collection and submission of data, providing cross-working and cover for the UKROC Coordinator

Person specification

Required

Essential criteria
  • Worked in the NHS or another larger organisation for over 2 years
  • Proficient and confident with Microsoft Office packages i.e. Access, Excel, Word
  • GCSE or equivalent level including Maths and English
  • Experience of managing own workload without close supervision and be able to multi-task within close deadlines and able to act independently
Desirable criteria
  • Knowledge and understanding of specialised rehabilitation
  • Experience and understanding of complex discharges and patients needs

  • Effective interpersonal and intrapersonal skills including high degree of self-awareness and self- regulation
Desirable criteria
  • Care experience with patients with traumatic brain injuries
