Job Title:Intensive Community Rehabilitation Team Nurse

An exciting opportunity has arisen for a full time enthusiastic, vibrant, Senior Staff Nurse, to work in our community Mental Health Rehabilitation Service.

The Intensive Community Rehabilitation Team is intended to offer an alternative for up to 50 service users, who historically may have utilised inpatient rehabilitation and other placements both locally and nationally. The team will have the capacity to offer intensive support for individuals in their existing homes or high-quality tenancies of their choosing.

The approach will embrace partnership working, whilst accessing community resources within a strong MDT approach. There will be a strong ethos on rehabilitation, recovery and promotion of independence.

The successful candidate will be working with a comprehensive staff team including a team manager who is an occupational therapist, psychiatrists, psychologists, occupational therapists and assistants , nurses , recovery workers and peer mentors to support the transformation agenda and implement an entirely new service.

Initially they may be involved in developing policies, establishing working practices, embedding the nurse role within the new team ,building relationships with other teams within the clinical pathway.
  • To be responsible for the assessment, planning implementation and evaluation of rehabilitation interventions including psychological, social financial, housing, recreational and employment support, this will include physical and mental health interventions for the team service users who will have a diagnosis of complex psychosis.
  • Responsible for the appropriate clinical risk assessment of service users contacting the “Out of Hours” service, providing an effective advice, guidance and decision-making process culminating in the coordination of any reviewed plan of care.
  • Responsible for the admission, case conference/reviews and discharge in collaboration with members of the multi-disciplinary team.
  • Responsible for participating in the development of the service, setting of quality standards, including the auditing, monitoring and reviewing in line with current clinical guidance practice and policy.
  • Responsible for ensuring the postholder understands and meets their professional responsibilities under the child protection legislation.
  • Responsible for ensuring that the requirements of the Mental Health Act 1983 are observed, adhered to and implemented
