Carelink Service Co-Ordinator
This may involve return to workmeetings after staff absence. Performance monitoring would include but not limited to: Adhesion to local and trust policies and guidelines, that apply to the service andensure that delivery of support meets the service standard Effective verbal communication with clients, families, colleagues and professionalsworking alongside Carelink Supporting enabling theory at all times for appropriate client Clear and accurate client records both written and electronic as required accordingto local policies Appropriate use of client filling system To coordinate the use of resources where required, eg Oyster cards, PPE 1.2 To supervise the shift and be the link person for the Community Health Support Workers(CHSW) by providing advice and support as and when needed1.3 When supervising the shift - to oversee the rostering of shifts and ensure scheduled visitsare delivered in order to meet the needs of the clients1.4 To take an active role in the appointed community locality when not undertaking the adminfunction by supervising shifts, conducting reviews, quality checks, competencyassessments and where required support and train staff1.5 To undertake visits to clients homes independently as the locality co-ordinator and ensurefirst visits assessments, risk assessments and care plan reviews are carried out in a timelymanner1.6 To take daily handover of clients progress from CHSWs and where appropriate reportback to the deputy manager/clinical co-ordinator and other staff members in the team orany change in clients condition or environment that may present health and safety and / orclinical issues for clients or other professionals1.7 To liaise with outside agencies such as Treating therapist, Adult Social Care, AdultCommunity Nursing and Specialist Palliative Care Team and hospital Accident andEmergency and Acute Medical Unit wards as required1.8 To monitor Carelink staff and ensure a high quality professional service is maintained i.e.adherence of uniform policy, health and safety, Infection control, information governanceand recording client progress on SystmOne1.9 To lead on orientation and induction of new members of staff, including assessing staffcompetencies through a set framework as guided by the Carelink clinical co-ordinator1.10 To understand and ensure the potholder and the team adheres to local and trust policiesand guidelines that apply to the service and ensure that delivery of support meets theservice standard1.11 To carryout audits and client feedback on the service by conducting Carelink qualityassurance questionnaires and friends and family test.1.12 To be the named key worker and establish, maintain effective communication with theclient, their family, referring teams
